Position Overview:
The Care Coordinator will work alongside physicians and perform services in accordance with the physician’s orders. Care coordinators are trained to perform administrative as well as clinical duties under the direct supervising of the attending physician.
 
Education/Experience:
  • Minimum Bachelor’s Degree (or similar) in medical/Health related studies such as Foreign Medical Graduate (MD), Foreign Nursing graduate, or an LPN.
  • Minimum 2 years of related experience in a clinic, medical office, or hospital setting
  • Knowledge of health care and managed care administration policies
  • Bilingual skills required (English/Spanish)
 
Duties and Responsibilities
  • Set up and maintains controls and documentation procedures
  • Update current problem list and past medical history for every patient visit. Perform all duties assigned as a medical assistant: Take vitals, administer vaccines, labs, EKG, Spirometry
  • Update medication list for every patient visit.
  • Analyze performance data and organizing electronic medical records in accordance with company and clinical standards.
  • Monitor performance by gathering relevant data and maintaining records
  • Ensure that completed screening tests, specialist notes, operative reports are filed correctly in the medical chart prior to patient consultation with the physician.
  • Screen medical chart prior to visit
  • Meet deadlines on reports and other assignments
  • Perform other duties as assigned
 
Skills
  • Knowledge of EHR (Electronic Health Records)
  • Use relevant information and individual judgment to determine whether events or processes comply with laws, regulations, or standards
  • Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines and standard accepted practices
  • Must develop constructive and cooperative working relationships with others
  • Understand written sentences and paragraphs in work related documents
  • Resourcefulness in problem solving
  • Strong written and verbal communications skills
  • Knowledge of medical terminology
  • Must be extremely detail oriented. Strong organizational and interpersonal skills
  • Must develop constructive and cooperative working relationships with others
 
Work Schedule
  • 8am-5pm M-F and an occasional Saturday; scrubs will be provided
 
Compensation
  • $22 per hour based on 40 hours per week ($46K annual salary, benefits, PTO, and more)
